Employees Provident Fund Division


The Employees Provident Fund division was established in 1959 to facilitate administration of the Employees Provident Fund Scheme (EPF) that came into operation consquent to the enactment of Employees Provident Fund Act No.15 OF 1958.

The EPF shceme basically covers almost all the employees in the private sector and the corporation sector. The main objective of this scheme is to provide social security or protection to employees at old age invalidity, or their dependents after dealth.

This is a contributory social security scheme where the employees and employers contribute 8% and 12% respectively, of the monthly total earnings of the employee.

The Commissioner of Labour is responsible for the general administration of the scheme, while Monetary Board of the Central Bank of Sri Lanka is responsible for the mantenance of individual accounts of the members and investment of funds.

As at 2003 there were around 50,880 active employers and 1,872,073 active employee members attached to the Fund.


The main activites of the EPF Division are:

•  Registrtation of employers and employees
•  Monitor remitances
•  Recovery of contributions from defaulters
•  Take appropriate legal action when the employers fail to comply the provisions of the law.
•  Determination of payment of benefits.
•  Ensure eligibility and recommend granting of housing loans to members under the Housing    Loan shceme.

Prescribed forms mainly for remitens contributory to the fund, claim for refund as well as general insturestions are given below.

The type of forms to be used as prescribed under the Act;

•  “A” form: for employee's details
•  “B” form: for certify the membership
•  “C” form: for inform employee's contributions to the Central Bank
•  “D” form: the perticulers of employees
•  “E” form: notification of termination of service by employee
•  “F” form: notification of termination of service by employer
•  “G” form: Declearation on E.P.F. membership by employee
•  “H” form: for nomination on behalf of the member
•  “I” form: Revocation of nomination
•  “J” form: nomination a name for employee
•  “K” form: Application form for EPF benefit claim
•  “L” form: Application form for EPF benefit claim of a deceased memeber
•  “M” form: Medical certificate
•  “N” form: Statement of contributions to the fund
•  “O” form: Declearation of contributions
•  “P” form: Notice of Prosecution
•  “Q” form: Application for refund money

Claim for EPF benefits after cessation of employment on reaching the retiring Age: (Section 23 (a) of EPF Act)

Male : Over 55 years
Female : Over 50 years


Following Documents are required


1. ”K” Form
2. Membership Card (“B” Card)
3. Original Birth Certificate
4. Photo Copy of the National Identity Card certified by the respective employer

5. Photo Copy of the certificate of Registration issued under the Indo – Ceylon Implementation)     Act No.14 of 1967 which is certified by the employer.
6. Certified copy of your bank account. (Pass Book)

Claim for benefits by a female Member cessation of employment in consequence of Marriage: (Section 23(b) of the Act)


Eligibility

  1. Employees who he is married with in three months from the date of resignation.

      Or

  1. Married Employees who have resigned from service with in a period of five years from the date of marriage.

The following documents are required.

1.  “K” From
2.  Membership card ( ‘B' Card)
3.  Original certificate of marriage
4.  A photocopy of your National Identity Card certified by the respective employer.
5.. Certified copy of your bank account. (Pass Book)

•  Leaving Employment due to Permanent Incapacity: (Section 23(c) of the Act)

1.  “K” Form
2.  Membership Card (“B” Card)
3.  “M” form
4.  The “Health 307” should be duly completed by a Registered Government Doctor.
5.  A Photocopy of National Identity Card certified by the employer.
6.  Certified copy of bank account. (Pass Book)

•  Applicants who migrate for permanent residence abroad should submit their applications directly to the E.P.F. Secretariant, No 356, Galle Road , Colombo 03.:(Section 23 (d) of the Act)

Following Documents are required

  1. “K” Form
  2. E.P.F. ”B” Card (Membership Card)
  3. Permanent Residence Visa
  4. Passport
  5. A Photocopy of National Identity Card certified by the employer.
  6. Certified copy of bank account. (Pass Book)
  • Leaving employment to join Public service or Local Government Service as permanent and pensionable employees. (Section 23 (e) of the Act)

    Following documents are required.

1.  “K” From
2.  Membership card (‘B' Card)
3.  Original letter of appointment
4.  Letter from the Head of the present Department specifying that the applicant is employed in      a permanent and pension able post.
5.  Applicants who are employed in the police or the Defense Forces, the respective service      card is required to be submitted together with a photocopy of the same.
6.  A Photocopy of National Identity Card certified by the employer.
7. Certified copy of bank account. (Pass Book)


Leaving Service on a Government Cooperation / Board being close down or on retrenchment or converted in to Company. (Section 23(f) of the Act)

Following documents are required.

1.  “K” From
2.  Membership card (‘B' Card)
3.  Letter from the Company
4.  A Photocopy of National Identity Card certified by the employer.
5. Certified copy of bank account. (Pass Book)

Following procedures should be taken in case of the member who have obtained a loans under the EPF Housing Loan Scheme .  

•  In the event of the member had obtained a housing loan, the duly certified “Loan form 14” which is issued by the District Labour Office (in 5 copies) certified by the relevant Bank (or lending institution) which was issued the loan and the Assistant Commissioner of Labour should be attached.
•  Please also note that the claimants should maintain an account in the Bank which was issued the housing loan. The legible photocopy of the bank account pass book also needs to attach to the application.

Refund of benefits of a deceased member: (Section 24 of the Act)

The applicant / applicants who are eligible to claim for EPF benefits of deceased member, are required to complete from ‘L” (with form L2 and L3) and hand over with other necessary document to the EPF Division of the Department of Labour (forms freely available in the Labour Office)

The following documents are required

1.   “L” form / if the claims are made more than one person, each applicant should submit “L”        application separately.
2.   Original Death Certificate of the deceased member.
3.   If deceased member was married, his original marriage Certificate.
4.   Original Birth Certificates of the children of the deceased.
5.   Legal heirs who are entitle to claim for benefits of the deceased member if any, should       forward the duly completed “L” forms certified by the employer together with the originals       of their birth certificates. (If the establishment is closed, ‘L' form should be certified by the       assistant Commissioner of the District Labour Office, who is in charge of the area of the       work place.)

6.   Original death certificates of the deceased legal heirs if any also should be submitted. The       name of the registrar and the official frank should be available in the certificates.       (Certificates bearing the date stamp and the official frank are also accepted.)

7.   Membership Card (“B” Card) of the deceased.
8.   A Photocopy of National Identity Card certified by the employer

9.   Copies of Bank Account Pass Books of Applicants.

Following procedures should be followed in case of the member who have obtained a loans under the EPF Housing Loan Scheme.

(iii) In the event of the member had obtained a housing loan, the duly certified “Loan form 14”       which is issued by the District Labour Office (in 5 copies) certified by the relevant Bank       which the loan was issued and the assistant commissioner of Labour should be attached.       Copies

(iv) Please note that the claimants should maintain an account or newly open an account in the       Bank by which the housing loan was issued to the member. The legible photocopies of such       bank accounts also be attached to the application.
